1. Call 724-4271 or 724-3320 to schedule a tour and presentation of EXPLORATIONS programs. There are no walk-in visitors allowed.

REGISTRATION

1. Enrolling families submit the following:
   
   • accomplished Parent Questionnaire
   • accomplished Student Information Sheet with two current 2x2 pictures
   • accomplished Fetcher’s Form with two current 1x1 pictures of each fetcher
   • photocopy of the child’s birth certificate
   • current Medical Form accomplished by child’s pediatrician
   • signed agreement with EXPLORATIONS Guidelines and Policies
